Registration

Welcome to the Registration page!  All students who will attend school in Bellwood School District 88 must complete registration online. This includes students who are new to the Bellwood area, students that have previously attended school in our district, and incoming pre-kindergarten and kindergarten students.

Each section below provides information regarding the registration process for the 2023-2024 school year.  
    • All parents and guardians of returning students will receive an email from “[email protected]” to the primary email address listed in PowerSchool for EACH STUDENT IN THE HOUSEHOLD. Please check your spam or junk folder if you cannot find the email in your inbox. 
    • The email will contain a 15-digit “Snapcode” that must be entered to access the students’ registration account.  EACH STUDENT IN THE HOUSEHOLD will receive their own "Snapcode". 
    • There will be a link in the email to log into the Online Registration (Infosnap) account to register your student(s).

  • I forgot my credentials: 
        • Select "Forgot Password?" to create a new password if you have forgotten your password.
        • Email [email protected] and ask for the email address we have on file for your student(s) if you have forgotten which email address you used previously. 
  • For parents and guardians without an email address on file, you will receive a letter in the mail for each student you have enrolled in the district.  Upon receipt, go to www.sd88.org and select ‘Registration’ from the ‘Parent Portal.’ Select the Returning Student link, enter your student’s SnapCode as it appears on the letter.  Once entered, you can proceed with the registration for your student.
  • An account must be created in order to begin the registration process. Creating an account allows parents and guardians to save their progress and return to the registration form as needed.  Parents and guardians only need to create one account.  The same account can be used for registering multiple students in the district.
 

Once you have logged in to your Infosnap account, please verify and update all pre-populated information. For all families that have changed residency since the previous school year, residency verification needs to be completed.  Review the list of acceptable documentation and upload the appropriate documents in the registration application. If your residency has not changed, you are not required to complete residency verification.

Once all information has been verified and the necessary documents uploaded, submit the application.  If the submit button is grayed out, it means that a mandatory field on the application has either been left blank or a date or phone number is in the incorrect format.  Thoroughly review your information and try to submit the application.  The system will not accept the application unless all sections are properly completed.

For incoming and returning 6th Grade students:  As determined by the Illinois State Board of Education (ISBE) and the Illinois Department of Public Health (IDPH), a new physical exam on the state’s physical form (dated within one year of the start of the school year) and an updated immunization record must be provided at the time of registration or before the state-mandated exclusion date. All required physical and immunization forms are due no later than October 13, 2023.  NOTE: ISBE does not allow any school to accept doctor’s appointments to avoid exclusion. Important Note: If an updated lease or residency documents are not available at the time you wish to re-enroll your student(s), you can still submit the online registration. However, your student(s) will not be placed in a class with a schedule generated until the required documents are submitted to the school.

THE 2023-2024 NEW STUDENT REGISTRATION PORTAL OPENS JUNE 1, 2023

All new and transfer students registering for the first time in Bellwood School District 88 must complete the registration online.  The New Student Portals can be accessed using the appropriate links in the side panel of this page.  Be sure to select the appropriate school for your student(s).  Our District Map can be viewed in the side panel of this page.  New student applications cannot be submitted unless all the appropriate documentation is uploaded into the Infosnap account.

All incoming students will be required to provide the most recent immunization record, physical examination and eye examination at the time of registration.

For incoming pre-kindergarten, kindergarten and sixth grade students, the following are required by ISBE: a new physical exam on the state mandated form (dated within one year of the start of the school year); an updated immunization record provided at the time of registration or before the state mandated exclusion date.  NOTE: Registration will be delayed for students that fail to  submit the required physical exam and updated immunizations at the time of registration.  Any student that fails to submit required medical forms will be excluded from school on October 13, 2023. Notice will be provided prior to exclusion.

The Illinois State Board of Education (ISBE) does not allow any school in the district to accept doctor’s appointments to avoid exclusion. To see the full list of health requirements, please see the Health Requirements section below.  All Health forms are located in the side panel of this page.  Parents must upload all documentation in the appropriate location in the registration application in order to submit the registration application.

Required Documents for New Students:

Category I (one of the following documents):

  • Parent - Guardian's Driver's License
  • Parent - Guardian's State ID
  • Parent - Guardian's Passport with Photo
  • Parent - Guardian's Photo ID from Local Consulate with Current Address

Category II (three of the following documents):

  • Gas, Electric, Water, Telephone, or Real Estate Tax Bill
  • Rental or Purchase Contract (Lease) or Mortgage
  • Insurance Policy (Renters or Homeowners)
  • Bank Statement (for proof of income)
  • Payroll or Child Support Check with Current Address

Important Information for New Students:

  • Contact your school to ensure the registration application(s) for your student(s) was received by the school.
  • Make an appointment with the secretary at your school if there are issues with documents missing from your registration application and to complete a Records Release Form for the previous school records. 
  • The Illinois Transfer Form is required for all new students coming from another Illinois school and or entering first through eighth grade.  Parents and Guardians must contact the previous school to retrieve this form prior to setting an appointment with the school.
  • Families that are experiencing hardships, or have unique circumstances that affect entry into school, please reference the Student Services page.
 

The parent(s)/guardian(s) of a student who is transferring from an Illinois public school shall ensure that a completed Transfer Form arrives at the school into which the student is transferring. 105 ILCS 5/2-3.13a.

The Transfer Form, ISBE Form 33-78, available at: www.isbe.net/Documents/33-78_student_transfer.pdf, indicates whether the student’s medical records are current and whether the student is currently being disciplined by a suspension or expulsion. 105 ILCS 5/2-3.13a(b).

If the student is transferring from any private school (whether located in Illinois or not) or any non-Illinois public school, the parent(s)/guardian(s) shall certify in writing that the student is not currently serving a suspension or expulsion imposed by the school from which the student is transferring. 105 ILCS 5/2-3.13a(b).

The Building Principal or designee shall deny admittance to any student who was suspended or expelled for any reason from any public or private school in this or any other state until the student completes the entire term of the suspension or expulsion. 105 ILCS 5/2-3.13a(a).

If the student is transferring from an Illinois Public School, the Building Principal or designee shall refuse to admit the student unless the student can produce a Transfer Form.

If the student is transferring from any private school (whether located in Illinois or not) or any non-Illinois public school, the Building Principal or designee shall refuse to admit the student unless his or her parent(s)/guardian(s) certify in writing that the student is not currently serving a suspension or expulsion imposed by the school from which the student is transferring.

 

All Early Childhood students, new and returning, are required to follow the same registration process as mentioned in the sections above. We accept pre-kindergarten students throughout the Proviso Township and surrounding areas.

Transportation, however, is limited to only those students within the School District 88 boundaries. Please contact the Early Childhood directly for an additional enrollment questions and to set up a preschool screening by calling (708)410-3701.

If you reside with a Bellwood, Broadview, Hillside, Maywood, Melrose Park or Stone Park resident, but are not the leaseholder or homeowner, you must provide a signed and notarized “Lives with Bellwood, Broadview, Hillside, Melrose Park, or Stone Park” letter, along with your required proof of residency documents.

Acceptable Proof of Residency Documents:

A total of four documents for new students and three documents for returning students for proof of residency are required as outlined below, one item from Category I and three items from Category II. Please contact the registration staff if you are having trouble collecting required documents.

Category I:

In cases of financial hardship, fixed income, or other similar circumstances, parents and guardians may request a fee waiver.  If you are experiencing a financial constraint, complete and submit the fee waiver form and upload the required documentation. After your form has been received and reviewed, a district office staff member will notify you regarding waiver approval.

In order to complete the fee waiver form, you will need a Google (Gmail) email address. If you do not have a Google (Gmail) email account, you may create one free at Gmail.

Illinois State Board of Education and Bellwood School District 88 require all students entering preschool, kindergarten and sixth grade to provide a new physical examination and updated immunization records dated within a year of the start of the school year.  Physical exams must be on the state approved form.

To ensure adequate protection and a positive start to the new school year for all of our students, please submit all required physical examinations and immunizations when submitting your online registration.  If the registration application has been submitted without the required health forms, submit them to your child's school as soon as possible.  Registration will be delayed for students that fail to  submit the required physical exam and updated immunizations at the time of registration.  Any student that fails to submit required medical forms will be excluded from school on October 13, 2023.  NOTE: ISBE does not allow any school to accept doctor’s appointments avoid exclusion.  All Health forms are located in the side panel of this page.

Students transferring into Bellwood School District 88 from another Illinois school district must submit their most recent physical examination and immunization records before the student is allowed to enroll. Students that are noncompliant may have to update their medical records before registration is completed.

Students transferring to Bellwood School District 88 from an out of state school district must submit a new physical examination and updated immunization record dated one year within the year of enrollment. Out of state transfer students are given a 30-day grace period to comply with medical requirements.

Students in Sports:

Students in 7th and 8th grade that wish to participate in one of the available sports (including cheerleading) are required to submit a sports physical prior to tryouts. Sports physicals are only valid for 12 months; therefore, a new one is required each school year.

Health Requirements and Physicals:

  • Students transferring into Bellwood School District 88 from another Illinois school district must submit their most recent physical examination and immunization records before the student is allowed to enroll. Students that are noncompliant may have to update their medical records before registration is completed.
  • Students transferring to Bellwood School District 88 from an out of state school district must submit a new physical examination and updated immunization record dated one year within the year of enrollment. Out of state transfers are given a 30-day grace period to comply with medical requirements.
 

The Illinois State Board of Education requires proof of a vision exam for all kindergarten students and any student transferring into the district, for the first time. Vision exams must be documented on the Illinois Department of Public Health vision form. Parent(s)/Guardian(s) must present proof of their child/children having been examined by an ophthalmologist or an optometrist by October 15 of the current school year. A waiver may be completed for any qualifying student, please contact the nurse at your child’s school for more information on the waiver.  All Health forms are located in the side panel of this page.

The Illinois State Board of Education requires proof of a dental exam for all students entering kindergarten, 2nd, and 6th grade. Dental exams must be documented on the Illinois Department of Public Health dental form.

Parent(s)/Guardian(s) must present proof of their student(s) dental examination by May 15 of the current school year. A waiver may be completed for any qualifying student, please contact the nurse at your child’s school for more information on the waiver.  All Health forms are located in the side panel of this page.

Children of parents or legal guardians who object to health, dental, or eye examinations, or any part thereof, or to immunizations or to vision and hearing screening tests on religious grounds shall not be required to undergo the examinations or immunizations if the parents or legal guardians present to the appropriate local school authority a signed Certificate of Religious Exemption detailing the grounds for objection and the specific immunizations and/or examinations to which they object.  The grounds for objection must set forth the specific religious belief(s) that conflict with the examination, immunization, or other medical intervention. 

 

The certificate will be signed by the parent or legal guardian to confirm their awareness of the school’s exclusion policies in the case of a vaccine-preventable disease outbreak or exposure.  The certificate must also be signed by the child’s health care provider responsible for performing the child’s examination for entry into preschool, kindergarten, or sixth grade.  The religious objection provided need not be directed by the tenets of an established religious organization.  However, general philosophical or moral reluctance to allow physical examinations, eye examinations, immunizations, vision, and hearing screening or dental examinations will not provide a sufficient basis for an exception to statutory requirements. 

 

The local school authority is responsible for determining if the content of the Certificate of Religious Exemption constitutes a valid religious objection.  The local school authority shall inform the parent or legal guardian of exclusion procedures in accordance with IDPH’s rules, Control of Communicable Diseases Code (77 Ill. Adm. Code 690) at the time the objection is presented.  The form can be found in the side panel of this page under "Health Forms."

Hearing screening must be provided annually for preschool children three years of age or older in any public or private educational program or licensed child care facility, and for all school-age children grades kindergarten, first, second and third; are in special education class; have been referred by a teacher, or are transfer students.  These screening services shall be provided in all public, private, and parochial schools. In lieu of the screening services required, a completed and signed report form, indicating the child had an ear examination by a physician and an audiological evaluation completed by an audiologist within the previous 12 months, is acceptable.

 

Vision screening must be provided annually for preschool children three years of age or older in any public or private educational program or licensed child care facility, and for school-age children in kindergarten, second and eighth grades; are in special education class; have been referred by a teacher, or are transfer students.  Such screening services shall be provided in all public, private, and parochial schools. In lieu of the screening services required, a completed and signed report form, indicating that an eye examination by a doctor specializing in diseases of the eye or a licensed optometrist has been administered within the previous 12 months, is acceptable.

 

The parent or legal guardian of a student may object to hearing or vision screening tests for their children on religious grounds. If a religious objection is made, a written and signed statement from the parent or legal guardian detailing such objections must be presented to the local school authority.

 

 

Students should not take medication during school hours or during school-related activities unless it is necessary for a student’s health and well-being.  When a student’s licensed health care provider and parent/guardian believe that it is necessary for the student to take medication during school hours or school-related activities, the parent/guardian must request that the school dispense the medication to the child and otherwise follow the district’s procedures on dispensing medication.  No school district employee shall administer to any student, or supervise a student’s self-administration of, any prescription or non-prescription medication until a completed and signed school medication authorization form is submitted by the student’s parent/guardian. 

 

No student shall possess or consume any prescription or non-prescription medication on school grounds or at a school-related function other than as provided for in this policy and its implementing procedures.  All medication must be delivered to the school by a parent or guardian in a container properly labeled by the pharmacy.  At the end of the school year, all medications must be picked up by an authorized adult.  No medication other than inhalers will be given to the student at the end of the school year.

 

A student may possess an epinephrine auto-injector (EpiPen®) and/or medication prescribed for asthma for immediate use at the student’s discretion, provided the student’s parent/guardian has completed and signed a School Medication Authorization Form. The school district shall incur no liability, except for willful and wanton conduct, as a result of any injury arising from a student’s self-administration of medication or epinephrine auto-injector or the storage of any medication by school personnel. A student’s parent/guardian must indemnify and hold harmless the school district and its employees and agents, against any claims, except a claim based on willful and wanton conduct, arising out of a student’s self-administration of an epinephrine auto-injector and/or medication, or the storage of any medication by school personnel. Nothing in this policy shall prohibit any school employee from providing emergency assistance to students, including administering medication.

 

The building principal shall include this policy in the student handbook and shall provide a copy to the parents/guardians of students. Please contact the nurse assigned to your child’s building for medication administration procedures.
