Welcome to student online registration for Bellwood School District 88. All students who will attend Bellwood School District 88 schools for the 2022–2023 school year must register online. This includes students who are registered for the current school year, incoming pre-k or kindergarten students, and any student new to BSD88. Please click the section below that pertains to the student you would like to register. We look forward to receiving your student’s enrollment.
We will send an email to the parent or guardian who we have listed in our system as the primary contact. The parent/guardian listed will receive an email from noreplyregistration@powerschool.com for each child already enrolled in the district. This will likely be the parent or guardian who originally registered the child, unless otherwise specified. There will be a link in the email that will allow you to log into your Online Registration account to register your child. If this is your first time completing an Online Returning Student Registration account, you will need to create an account. This will allow you to securely save your work and come back at a later time. If you already have an account, you can sign in and complete the form. You should use this same account to complete forms for multiple children. If you have forgotten the password, select "Forgot Password?" If you have forgotten the email address used for this account please email registration@sd88.org and ask which email you provided as your email address for this account.
For parents and guardians without an email address on file, you will receive a letter in the mail for each child you will be enrolling. Upon receipt, go to the Registration page, click on Returning Students and click the Online Registration – Returning Student link. Then, enter your student’s snapcode as it appears on the letter. The snapcode is like a key to your child's information for the upcoming school year. You will receive a unique snapcode for each of your children.
For incoming 6th Graders: It is important for you to know the legal health requirements for students entering 6th grade, as determined by the Illinois State Board of Education and the Illinois Department of Public Health. A new physical exam on the state-mandated form (dated within one year of the start of the school year) and an updated immunization record must be provided at the time of registration or before the state-mandated exclusion date. All required physical and immunization forms are due no later than October 15, 2022. This communication allows families ample and advanced opportunity to meet these legal requirements and exclusion will not occur without prior notice. Please note the Illinois State Board of Education does not allow us to accept doctor’s appointments after October 15 to avoid exclusion. Verify Information and Upload Residency Documents
Once you have logged in to your Infosnap account, please verify, or update any pre-populated information. For all families that have changed residency since the previous school year, we need you to complete residency verification. Please review the list of accepted documentation and upload the appropriate residency verification documents. If your residency has not changed, you are not required to complete residency verification.
Important: If you will not have your updated lease or residency documents at the time you wish to re-enroll, you can still complete the online registration. However, we will not place students on class lists or have middle school schedules generated until their families have finished the online registration and submitted their proof of residency. Please complete this process by or before August 15.
Submit: Once you have verified the information and uploaded any necessary documents, hit the submit button to complete the application. Your student is not considered re-enrolled until you click submit. If the button is greyed out for you, it means that a mandatory field on the application has either been left blank or a date or phone number is in the incorrect format. Please be sure to thoroughly review your information before completing the application because you will not be able to make changes to the form after you submit it.

All new students transferring in or registering for the first time in District 88, will be required to provide the most recent immunization record and physical examination at the time of registration.
For incoming pre-kindergarten, kindergarten and sixth grade students, the following are required: a new physical exam on the state mandated form (dated within one year of the start of the school year); an updated immunization record provided at the time of registration or before the state mandated exclusion date. Unfortunately, the Illinois State Board of Education (ISBE) does not allow districts to accept doctor’s appointments after October 15. To see the full list of health requirements please click the button below. If you completed all requirements, please remember to upload all documentation during the registration process. Once you submit your registration, you will not be able to go back and upload your documents. The final step of the registration process is to contact the school that your student will be attending, schedule an appointment to deliver the required documentation in-person, and deliver the documentation at the scheduled time. The building staff at all schools returns on Monday, August 1, 2022.
Required Documents for New Registrations
We need you to provide the following documents to complete your child’s registration.
Category I: (one of the following documents)
- Parent - Guardian's Driver's License
- Parent - Guardian's State ID
- Parent - Guardian's Passport with Photo
- Parent - Guardian's Photo ID from Local Consulate with Current Address
Category II: (three of the following documents)
- Gas, Electric, Water, Telephone, or Real Estate Tax Bill
- Rental or Purchase Contract (Lease) or Mortgage
- Insurance Policy (Renters or Homeowners)
- Bank Statement (for proof of income)
- Payroll or Child Support Check with Current Address
These documents must be dated within the year of the first day of school, August 29, 2022, for students entering kindergarten, sixth grade, or an Illinois school for the first time. You will have until the first day of school to produce these documents. For more information, please review our Health Requirements for New Students.
- The Illinois Transfer Form is required for all new students coming from another Illinois school and or entering first through eighth grade. Please contact your previous school to retrieve this form prior to attending a District 88 registration event.
- Please be sure to complete the Records Release Form, during your scheduled in-person visit to submit the required documentation.

Students new to the Early Childhood Center, are required to follow the same registration process as new students to Bellwood School District 88. We accept preschool and pre-kindergarten students throughout the Proviso Township and surrounding areas. Transportation, however, is limited to only those students within the School District 88 boundaries. Please contact a member of the Bellwood Early Childhood Secretarial Team at (708) 410-3701 for an additional enrollment appointment and to set up a preschool screening.

The parent(s)/guardian(s) of a student who is transferring from an Illinois public school shall ensure that a completed Transfer Form arrives at the school into which the student is transferring. 105 ILCS 5/2-3.13a.
The Transfer Form, ISBE Form 33-78, available at: www.isbe.net/Documents/33-78_student_transfer.pdf, indicates whether the student’s medical records are current and whether the student is currently being disciplined by a suspension or expulsion. 105 ILCS 5/2-3.13a(b).
If the student is transferring from any private school (whether located in Illinois or not) or any non-Illinois public school, the parent(s)/guardian(s) shall certify in writing that the student is not currently serving a suspension or expulsion imposed by the school from which the student is transferring. 105 ILCS 5/2-3.13a(b).
The Building Principal or designee shall deny admittance to any student who was suspended or expelled for any reason from any public or private school in this or any other state until the student completes the entire term of the suspension or expulsion. 105 ILCS 5/2-3.13a(a).
If the student is transferring from an Illinois Public School, the Building Principal or designee shall refuse to admit the student unless the student can produce a Transfer Form.
If the student is transferring from any private school (whether located in Illinois or not) or any non-Illinois public school, the Building Principal or designee shall refuse to admit the student unless his or her parent(s)/guardian(s) certify in writing that the student is not currently serving a suspension or expulsion imposed by the school from which the student is transferring.

Families will need to upload all required documentation in the online registration system. You can use a computer, tablet, or smartphone. Please be sure to upload all required documents before clicking the submit button to complete the registration process.
All students are assessed a registration fee depending on their grade level. The fee schedule is as follows:
- Pre-K - No Fees
- K–5th Grade - $60
- 6th–8th Grade - $75
In cases of financial hardship, fixed income, or other similar circumstances, parents and guardians may request a fee waiver. Please complete and submit the 2022–2023 fee waiver form and upload the required documentation. After your form has been received and reviewed, we will notify you regarding waiver approval.
In order to complete the fee waiver form, you will need a Google (Gmail) email address. If you do not have a Google (Gmail) email account, you may create one free at Gmail.
Registration for Families With Inconsistent Residency
- Returning students with special needs: Please register using the online portal.
- Returning students with special needs attending schools outside of district: Please use the online portal and (x) the pay later box.
- Registration for new students with special needs must register in person.

A total of four documents for new students are required as outlined below, one item from category I and three items from category II.

Physicals and Immunizations
Illinois State Board of Education and Bellwood School District 88 require all students entering preschool, kindergarten and sixth grade to provide a new physical examination and updated immunization records dated within a year of the start of the school year. Physical exams must be on the state approved form. A copy of the state form can be found at the following link:
To ensure adequate protection and a positive start to the new school year for all of our students, please submit all required physical examinations/ immunizations to your child's home school as soon as possible and make certain to keep a photocopy of all submitted exam forms for your own records. Students that fail to submit the required physical exam forms will be excluded from school on October 15. Please note Illinois State Board of Education does not allow school districts to accept appointments after October 15 to prevent exclusion.

Students in 7th and 8th grade that wish to participate in one of the available sports (including cheerleading) are required to submit a sports physical prior to tryouts. Sports physicals are only valid for 12 months; therefore, a new one is required each school year. Please click the link below to access the IESA Sports Physical Form.
Vision Exams
The Illinois State Board of Education requires proof of a vision exam for all kindergarten students and any student transferring into the district, for the first time. Vision exams must be documented on the Illinois Department of Public Health vision form (see link below). Parent(s)/Guardian(s) must present proof of their child/children having been examined by an ophthalmologist or an optometrist by October 15 of the current school year. A waiver may be completed for any qualifying student, please contact the nurse at your child’s school for more information on the waiver.
Dental Exams
The Illinois State Board of Education requires proof of a dental exam for all students entering kindergarten, 2nd, and 6th grade. Dental exams must be documented on the Illinois Department of Public Health dental form (see link below).
Parent(s)/Guardian(s) must present proof of their child/childrens dental examination by May 15 of the current school year. A waiver may be completed for any qualifying student, please contact the nurse at your child’s school for more information on the waiver.

Religious Exemptions
Children of parents or legal guardians who object to health, dental, or eye examinations, or any part thereof, or to immunizations or to vision and hearing screening tests on religious grounds shall not be required to undergo the examinations or immunizations if the parents or legal guardians present to the appropriate local school authority a signed Certificate of Religious Exemption (See link below) detailing the grounds for objection and the specific immunizations and/or examinations to which they object. The grounds for objection must set forth the specific religious belief(s) that conflict with the examination, immunization, or other medical intervention. The certificate will be signed by the parent or legal guardian to confirm their awareness of the school’s exclusion policies in the case of a vaccine-preventable disease outbreak or exposure. The certificate must also be signed by the child’s health care provider responsible for performing the child’s examination for entry into preschool, kindergarten, or sixth grade. The religious objection provided need not be directed by the tenets of an established religious organization. However, general philosophical or moral reluctance to allow physical examinations, eye examinations, immunizations, vision, and hearing screening or dental examinations will not provide a sufficient basis for an exception to statutory requirements. The local school authority is responsible for determining if the content of the Certificate of Religious Exemption constitutes a valid religious objection. The local school authority shall inform the parent or legal guardian of exclusion procedures in accordance with IDPH’s rules, Control of Communicable Diseases Code (77 Ill. Adm. Code 690) at the time the objection is presented.
Vision and Hearing Screenings
Hearing screening must be provided annually for preschool children three years of age or older in any public or private educational program or licensed child care facility, and for all school-age children grades kindergarten, first, second and third; are in special education class; have been referred by a teacher, or are transfer students. These screening services shall be provided in all public, private, and parochial schools. In lieu of the screening services required, a completed and signed report form, indicating the child had an ear examination by a physician and an audiological evaluation completed by an audiologist within the previous 12 months, is acceptable.
Vision screening must be provided annually for preschool children three years of age or older in any public or private educational program or licensed child care facility, and for school-age children in kindergarten, second and eighth grades; are in special education class; have been referred by a teacher, or are transfer students. Such screening services shall be provided in all public, private, and parochial schools. In lieu of the screening services required, a completed and signed report form, indicating that an eye examination by a doctor specializing in diseases of the eye or a licensed optometrist has been administered within the previous 12 months, is acceptable.
The parent or legal guardian of a student may object to hearing or vision screening tests for their children on religious grounds. If a religious objection is made, a written and signed statement from the parent or legal guardian detailing such objections must be presented to the local school authority.
Medication Policy and Procedure
Students should not take medication during school hours or during school-related activities unless it is necessary for a student’s health and well-being. When a student’s licensed health care provider and parent/guardian believe that it is necessary for the student to take medication during school hours or school-related activities, the parent/guardian must request that the school dispense the medication to the child and otherwise follow the district’s procedures on dispensing medication. No school district employee shall administer to any student, or supervise a student’s self-administration of, any prescription or non-prescription medication until a completed and signed school medication authorization form is submitted by the student’s parent/guardian. No student shall possess or consume any prescription or non-prescription medication on school grounds or at a school-related function other than as provided for in this policy and its implementing procedures. All medication must be delivered to the school by a parent or guardian in a container properly labeled by the pharmacy. At the end of the school year, all medications must be picked up by an authorized adult. No medication other than inhalers will be given to the student at the end of the school year.
A student may possess an epinephrine auto-injector (EpiPen®) and/or medication prescribed for asthma for immediate use at the student’s discretion, provided the student’s parent/guardian has completed and signed a School Medication Authorization Form. The school district shall incur no liability, except for willful and wanton conduct, as a result of any injury arising from a student’s self-administration of medication or epinephrine auto-injector or the storage of any medication by school personnel. A student’s parent/guardian must indemnify and hold harmless the school district and its employees and agents, against any claims, except a claim based on willful and wanton conduct, arising out of a student’s self-administration of an epinephrine auto-injector and/or medication, or the storage of any medication by school personnel. Nothing in this policy shall prohibit any school employee from providing emergency assistance to students, including administering medication.
The building principal shall include this policy in the student handbook and shall provide a copy to the parents/guardians of students. Please contact the nurse assigned to your child’s building for medication administration procedures.
